In the novel The Body in the Library, Dr Metcalf is one of the best known physicians in Danemouth. He has no aggressive bedside manner, but his presence has a cheering effect. He is middle-aged, and has a quiet pleasant voice.
He says that Conway Jefferson is overworked, and needs to slow down, as over-exertion exacerbates his heart condition. Dr Metcalf has also warned Adelaide Jefferson and Mark Gaskell that Conway must not be over-excited or startled, as a sudden shock might kill him.
